Is whey protein safe? Kidneys, weight gain, acne and daily use

Short answer
For healthy adults, a meta-analysis of 28 trials found higher protein intakes did not change kidney function (GFR) compared with lower intakes, and the ISSN treats powder as a practical way to reach 1.4 to 2.0 g per kg a day. Whey does not make you fat by itself. Acne reports exist, as case reports in teenagers.
Is whey protein safe for your kidneys?
Devries and colleagues pooled 28 randomised trials with 1,358 healthy adults who did not have kidney disease, comparing higher protein intakes (at least 1.5 g per kg, or 20 percent of energy, or 100 g a day) with normal or lower intakes. The change in glomerular filtration rate (GFR), the standard measure of kidney filtering, did not differ between the two (standardised mean difference 0.11, 95 percent confidence interval from minus 0.05 to 0.27). The authors concluded that higher protein intakes do not adversely influence kidney function on GFR in healthy adults.
Three limits matter. The trials excluded people with kidney disease, they only had to run longer than 4 days to be included, so they say little about decades of use, and the authors flag unclear risk of selection bias in the included trials. If you have a kidney condition, ask a doctor before raising your protein. A 2016 review by Phillips, Chevalier and Leidy likewise wrote that they could find no evidence based link between higher protein diets and renal disease in healthy people.
Does whey protein make you fat?
Whey is protein, and it adds the calories printed on its label like any food. Body fat changes with energy intake over time. The ISSN diets and body composition position stand says diets aimed at fat loss are driven by a sustained caloric deficit, and diets aimed at gaining lean mass by a sustained surplus.
So a scoop on top of your normal eating adds calories, and a scoop that replaces something else may not. Check the label on the product page and count it. The ISSN protein position stand describes supplementation as a practical way to secure protein while minimising caloric intake.
Can whey protein cause acne?
One report, Silverberg in Cutis in 2012, described five male patients aged 14 to 18 whose acne started shortly after they began whey protein. Four cleared after they stopped, and one flared again when he restarted. At least six brands were involved. It is five case reports, which cannot show cause, and the author called for larger studies.
If you are acne prone and notice flares after starting a new powder, stopping for a few weeks is a reasonable test, and a dermatologist can advise. We found no larger study to quote, so we will not say more than the reports do.
Can you take whey protein every day?
The ISSN position is a daily intake of 1.4 to 2.0 g of protein per kg of bodyweight for most exercising people, from food or powder, and a general dose of 0.25 g per kg or 20 to 40 g per meal. Daily powder that fills a gap in that number sits inside the position. Daily powder on top of a diet that already meets it adds calories and nothing the position asks for.
Whey is a milk protein. If you are allergic to milk, it is not for you. For children and teenagers, ask a doctor or dietitian first: the ISSN position is about healthy, exercising individuals, and the acne reports above were all in teenagers.
